2010-2011 Pre-K Registration

This program is free and provides certified teachers and aides, transportation and strong parent involvement for any child who will be 4 years of age on or before December 1, 2010.

For further information, please call Mrs. Patricia Fallon at Annsville Elementary School, 334-3080.

After March 29th, you will need to contact our Central Registration Office @ 245-4096 to register.

Pre-K Coverlet (pdf file)
This form must accompany the registration packet as there is data required to be completed. This form is generally an 11 x 17 wraparound cover. It will print here as 2, 8-1/2 x 11 forms - which is acceptable.
Pre-K Registration Packet (pdf file)
This is the registration packet required to enroll in the Pre-K program. This will accompany the coverlet pages.
Literacy Survey (pdf file)
When downloading the forms to register for Pre-K, please also download the survey. Complete the survey and include with your completed registration packet.
