If you only have changes to the pick up and/or drop off points for your child, [change in babysitter, etc.] this form is used to make those changes.  If you have a change of address, please refer to the applicable section for this type of change.

The district has a 48-hour processing timeframe for establishing bus transportation.  Parents may need to transport their child during this period.

IMPORTANT REMINDER:
   On an annual basis you will be required to update this information.

Each school year the transporation department uses your home address as the starting point for pick up and drop off.  Babysitter pick-up and drop-off points need to be re-established each school year.
